Reporting Unemployment Fraud and Identity Theft

 
To help speed up the process, the Severance Police Department asks you to download a digital copy of the Police Department’s Identity Theft Packet.
 
The Identity Theft Packet can be completed on a computer or by hand. Once completed, you may print a copy for the responding Officer or email it to the Severance Police Department.
 
Identity Theft Packets emailed to the Severance Police Department will take several days to process. Please provide a Cell phone number in the email so the Officer can call you with the case report number.
 
Please follow the instruction in the Identity Theft Packet. If you have any questions, please call a Severance Police Officer for help.
 
Once the Severance Police Department has received a completed Identity Theft Packet, a Severance Police Officer will give you the case report number.
